So many women tell me the same story. They’re trying so hard to do everything right, and then the sweet tooth wins. Then they start Zepbound, and things change.

One of my clients couldn’t believe it. Her husband brought home a box of chocolates, and seven days later it was still sitting in the kitchen. Before Zepbound, a box like that never lasted more than a couple of days.

Zepbound makes it easier to eat less. This plan makes sure that even when you eat less, you still get what your body needs during perimenopause and menopause.

What Makes This Plan Different

I built this plan with the needs of women in perimenopause and menopause in mind. It focuses on getting enough fiber and eating your foods in an order that’s kinder to your blood sugar. Wherever possible, the food examples are anti-inflammatory choices.

Build Your Meals

The plate method is still one of the easiest ways to eat on Zepbound. Use any 9-inch plate, and eat it in this order:

  1. Vegetables
  2. Protein
  3. Carbs

Starting with the fiber in your vegetables and saving carbs for last helps your blood sugar after the meal.

Vegetables and fruit, a little less than ½ of your plate. Start here.

Aim for five servings a day, two fruits and three vegetables, and around 25 grams of fiber a day. They add fiber, vitamins, and volume for very few calories. Fiber also helps with constipation, a common side effect. If your stomach is sensitive in the first weeks, add fiber slowly.

Examples include:

  • Leafy greens: spinach, arugula, collard greens, kale, romaine
  • Cruciferous vegetables: broccoli, cauliflower, Brussels sprouts, red and green cabbage
  • Deep yellow and orange vegetables: carrots, butternut squash, orange and yellow bell peppers
  • Other vegetables: tomatoes, mushrooms, zucchini, green beans
  • Fruit: all berries, oranges, mandarins, grapefruit, peaches, plums, pomegranates, watermelon, apples

Protein – ¼ of your plate. Eat it next.

That’s a little more protein than the usual healthy plate, which gives protein a quarter. While you’re losing weight on Zepbound, you need more protein to protect your muscle.

On Zepbound you may only get through half your plate. A few bites of vegetables, then your protein, means you’ve covered the fiber and the part that protects your muscle before you get full.

Examples include:

  • Poultry: chicken or turkey breast
  • Fish and seafood: salmon, tuna, shrimp, white fish
  • Lean meats: lean beef, pork tenderloin
  • Eggs
  • Dairy: Greek yogurt, cottage cheese, low-fat cheese, milk
  • Soy: tofu, tempeh, edamame
  • Beans and lentils (these count as a protein and a carb)
  • Protein shakes or powder, for days when eating is hard

Whole grains and starchy carbs, 1/4 of your plate.

This is where being mindful of carbs comes in. Keep them to a quarter of the plate, choose whole grains for the fiber, and pick versions with less added fat, salt, and sugar.

Examples include:

  • Whole grains: oats, quinoa, brown and wild rice, barley, farro
  • Bread and pasta: whole wheat bread, whole grain tortillas, whole wheat pasta
  • Starchy vegetables: sweet potatoes, potatoes, corn, green peas, yuca, plantain
  • Legumes: all beans, chickpeas, lima beans, and lentils (these count as a carb and a protein)

Healthy fats

Use fat in moderation. It adds flavor, helps you feel satisfied, and helps your body absorb some vitamins, but fatty and fried foods can make nausea worse on Zepbound.

Examples include:

  • Oils: extra virgin olive oil, avocado oil
  • Avocados
  • Nuts: almonds, cashews, hazelnuts, pecans, pistachios, walnuts
  • Seeds: chia seeds, flaxseed, hemp seeds, pumpkin seeds
  • Nut butters: peanut butter, almond butter
  • Fatty fish: salmon, sardines, herring, mackerel (these count as a protein too)

My Expert Opinion

Zepbound can completely change the way you eat. But not everyone reacts the same way, and your own body will change along the way too.

How you feel when you first start, or right after a dose increase, can be very different from how you feel after a few weeks on the same dose. Your appetite will be lower, but most of the time it won’t be so low that you can’t eat at all. Some days you’ll want very little. Other days you’ll feel closer to normal.

Whatever your appetite is doing, the focus stays the same: eat in a way that gives you the most nutrients, and think high protein and high fiber every time.

  1. I have ibs-d and GERD also can’t have nuts or lettuce or anything that causes gases going on zepbound the dr said i have to change out the fiber due to my stomach issues need nutricional advise of what diet to follow please help

  2. Hi. This is great info. I am waiting for my pharmacy to fill my prescription so I should be starting Zepbound within the week. Please clarify the amount of food.
    On the PDF, it indicates that a fourth of the plate should be protein however, in your write up in the first part of the blog, it says 1/3 of the plate.

  3. I recently started Zepbound and have read that drinking too much water can actually sabotaged results and lessen the slowing of the stomach. I drink 85oz per day is that okay while taking Zepbound

    1. The exact amount of water depends on your characteristics, but 85 ounces sounds about right. There are a few online water calculators, and your health care provider can tell you better, BUT a good test is the pee test. If your pee is clear, chances are you are drinking enough.
